Patient Navigator
The Black Maternal Health Center of Excellence provides perinatal wraparound services to Black women throughout Los Angeles County. We are seeking patient navigator(s) to provide mental, social and administrative support during prenatal and postpartum journey. Our Patient Navigators conduct monthly check-ins with their assigned clients where they evaluate stress levels and needs for additional support. This role reports all updates directly to a Clinical Lead (OBGYN, MD, or LM) and to the Lead Patient Navigator. Evidence-based tools and trainings are provided upon hiring.
Highlighted Duties and Responsibilities:
· Monthly check-in with assigned clients including completed surveys and updated care plan(s)
· Act as a liaison between clients and BMHCE staff
· Provide health education, as needed, using materials selected by Clinical Lead
· Provide case management to ensure all requested services and supports are fulfilled
· Administers surveys (EPDS, PHQ-9, etc) to participants to assess stress and social determinants of health
Minimum Qualifications:
· Background in maternal and child health, social work, or health sciences
· Previous experience as a Community Health Worker or providing case management, health education, or social services
· Ability to work independently with minimal supervision
· Commitment and ability to provide Black birthing families with an affirming experience.
